Completed and signed application form	Online-generated or paper form, signed by a parent or guardian.
Student’s ID document	HKID card or passport (valid, colour copy).
Parent/Guardian’s ID document	HKID, passport or Home Return Permit (colour copy of both sides).
School reports (last two academic years)	Must bear the school chop.
Conduct certificate (last one to two years)	Conduct grade B or above.
Immunisation record	HK students: DH6 Immunisation Card. Mainland students: Children’s Vaccination Certificate.
Recent passport-sized photo	One colour photo with white background (approx. 35 × 45 mm).
Co-curricular or award certificates (if any)	Certificates, portfolios or reference letters (optional).
Application fee receipt	No Application Fee required

Transfer students must also provide proof of enrolment from their current school.

Submission Methods

• Online: upload all copies through the application portal.
• By post or in person: Admissions Office, Zhongshan Whampoa Academy, 10 Tat Chee Avenue, Kowloon Tong.
